Green flags and red flags
This is the heart of choosing well. The best agencies share a handful of traits — and the ones to avoid tend to share the opposite.
Look for these
- Real med-spa experience. They understand medical advertising rules and before/after compliance — not just “we do dentists too.”
- Verifiable results. Case studies and numbers you can actually check, not just a wall of client logos.
- They practice what they preach. A marketing agency should market itself well. Do they rank for their own services?
- Transparent pricing. They tell you what's included and what costs extra — without you having to ask twice.
- You own your assets. Your website, Google Business Profile and ad accounts stay in your name, always.
- Plain-English reporting. You can see what you're paying for and what it's actually producing.
- Fair terms. Month-to-month or a short initial term — they earn the relationship, not cage you in it.
Walk away from these
- Guarantees. “Guaranteed #1 on Google” or guaranteed results. No one can promise that honestly.
- They hold your assets hostage. Your site or ad account is in their name, so leaving means starting from scratch.
- Vague pricing. You can't get a straight answer on what it costs or what's included.
- No med-spa experience. And no curiosity about your compliance obligations or your patients.
- All contract, no proof. A long lock-in and a big setup fee, with no verifiable results to back it up.
- Pressure and urgency. “This price is only good today.” Good agencies don't need to rush you.
- Cookie-cutter everything. The same template site and the same captions as ten other clinics.

Most pricing confusion comes from one thing: not separating the agency's fee from your ad spend.
Retainer vs ad spend
The agency's fee pays for their time and expertise. Ad spendis separate money that goes straight to Google or Meta. A “$3,000/month” agency might mean a $3k fee plus $2–4k of ads you also fund. Always ask which is which — it changes your real monthly number completely.
What it should cost
Most med-spa retainers land between roughly $2.5k and $8k+ a month depending on scope, with onboarding or setup fees common up front. Cheaper isn't automatically better — very low prices usually mean outsourced, cookie-cutter work. And match the model to your goal: ads work in weeks, SEO compounds over 6–12 months— don't judge one on the other's timeline.
