The findings
Nearly 9 in 10 won’t tell you the price
77% hide pricingOnly 15 of 64 agencies publish a starting price — the other 77% make you book a sales call first. Where pricing is public, managed retainers run from $199 to $18,500/mo (median ~$1,500). That opacity is the single biggest source of friction for an owner trying to compare options.
Most “med spa” agencies are generalists
Only 14% specializeJust 9 of 64 agencies work exclusively in aesthetics. The majority serve med spas alongside dentists, lawyers and contractors — “med spa marketing” is often a landing page, not a specialty. That matters: the compliance rules and patient psychology in aesthetics aren’t something you pick up on the side.
The tell: SEO agencies that can’t rank themselves
9 rank top-3 for nothingWe measured every agency’s own organic footprint. Of 57 selling SEO, 25 (44%) get under 500 organic visitors a month to their own website, and 9 rank in Google’s top 3 for exactly zero keywords. Each bar below is one agency’s monthly organic traffic, sorted — the field collapses from one giant down to single digits. An SEO agency that can’t rank itself is the loudest signal in the dataset.
81%
Contracts are a black box
Only 12 clearly offer month-to-month. The rest don’t state whether you’re locked in — and none publish a minimum term.
14%
Thin footprint of their own
9 of 64 have little or no Google reviews — some selling reputation management. The rated ones average 4.73★.
33%
Asking for blind trust
Show no case studies at all. With hidden pricing and undisclosed contracts, that’s a lot asked of an owner on faith.
How we did this
Computed directly from the 64 med spa marketing agencies published in the RankMedSpa directory as of June 2026. Business facts and ratings come from the official Google Places API; each agency’s own organic search footprint (ranked keywords, traffic, top-3 positions) is measured via licensed DataForSEO data; pricing, contract terms, specialization and services are recorded against one standardized checklist. Figures update as the directory grows — nothing is invented. See our full methodology.
